Fees

Payment for your initial evaluation is due at the time of booking. Laboratory testing costs typically start at approximately $100 and, depending on the type and extent of testing needed, may exceed $5,000. Most patients should also anticipate additional costs for pharmaceuticals and nutraceuticals, as recommended based on their individualized care plan.
Care Plan
Our team creates a personalized care plan based on your individual needs, including next steps, medications, lab recommendations, and lifestyle changes. A structured follow-up schedule with clear guidance is provided to ensure continuity of care.
Communicate
We ask that all current patients use the Patient Portal for all communications. Individuals interested in becoming patients may contact us by phone or email for additional information or clarification.
Testing Done
We will review relevant prior testing as part of your evaluation, when appropriate. If the testing was completed through laboratories we are familiar with and trained to interpret, results may be submitted to our office for review during your appointment.
Extensive lab review that cannot be performed during your appointment is charged at $250/hour.
Purchase Supplements
Mountain Roots partners with a select group of trusted supplement companies that have been carefully vetted for quality, safety, and clinical integrity. Supplement recommendations are individualized and may be included as part of your personalized care plan.
For current patients, recommended supplements and ordering instructions are available in your Patient Portal.
While we collaborate with leading supplement partners, patients are always welcome to purchase supplements from the provider of their choice. For a list of approved partners and direct ordering links, please visit the Partners tab.
Refills
For prescription refills, please send a message through the Patient Portal directly to your practitioner requesting the medication.
Please provide your provider with at least 7 days’ notice before you run out of your medication for refills processed through a local pharmacy. Failure to do so may result in a gap in treatment.
For mail-order or compounding pharmacies, please allow at least 14 days’ notice, as processing and shipping times may vary.
When requesting a refill, be sure to include the medication name, dosage, frequency, and pharmacy information.
For patient safety, we reserve the right to deny prescription refills until you have been evaluated by your practitioner.
Patients prescribed medications for chronic conditions—such as high blood pressure, diabetes, or bio-identical hormone therapy—are typically required to be seen every three months. This schedule may vary depending on the medication and the clinical discretion of your practitioner.
If you are taking medications for mood disorders or ADD/ADHD—including, but not limited to, long-term use of stimulants, benzodiazepines, anti-anxiety medications, or antidepressants—you may be required to have monthly visits and/or follow up with a prescribing psychiatric provider for medication management and monitoring.

Note: If you have not been seen by your practitioner in more than one year, an appointment will be required before any medication refills can be authorized.
Personal Info
We do not sell, distribute, or lease your personal information to third parties unless required to do so by law.
Your personal information may be used to communicate important updates, including changes to our policies, practice notices, newsletters, or occasional promotions. You may opt out of non-essential communications at any time.
We are committed to protecting your privacy and comply with all applicable privacy and security regulations, including the Health Insurance Portability and Accountability Act (HIPAA). All protected health information is handled in accordance with federal and state privacy laws.
You may request details of the personal information we maintain about you, as permitted by law. Requests may be subject to a reasonable administrative fee.

Do you accept insurance?
We are a cash-pay practice and do not bill insurance directly. However, we can provide a superbill upon request. Patients may submit to their insurance carrier for potential reimbursement.
Patients with out-of-network benefits may receive partial reimbursement depending on their individual insurance plan. It is the patient’s responsibility to verify insurance benefits and eligibility with their insurance provider prior to services rendered. Reimbursement is not guaranteed and is determined solely by your insurance carrier.
