Schedule Your New Patient Evaluation
You will then receive a comprehensive evaluation to complete.
When you close your eyes and imagine your healthy self, what does that look like?
I want to know so that I can help you achieve your goals.
This is when you get to tell me what’s important to you. What are your priority health concerns that you want to work on.

Your New Patient Evaluation is 90 minutes
During this visit I will review with you the comprehensive evaluation you completed.
You will have time to tell the story of your health and life.
Together we will start to develop your personalized plan to help you achieve your health goals.
4.
Follow Up Appointment and Plan: Typically 90 minutes
Your first follow up appointment is scheduled for 2-4 weeks after your initial appointment.
Here you will have an opportunity to disucss and fine tune your personalized treatment plan, check in on your porgress, and discuss any roadblocks you face.
